GENERAL MEDICINE CASE DISCUSSION.

This is online E-blog, to discuss our patient de-identified health data shared after taking her guardian's signed informed consent. Here we discuss our individual patient problems through series of inputs from available global online community of experts with an aim to solve the patients clinical problem with current best evidence based input. This E-blog also reflects my patient's centred online learning portfolio. I have been given this case to solve in an attempt to understand the topic of "Patient Clinical Data Analysis" to develop my competency in reading and comprehending clinical data including history, clinical findings, investigations and come up with a diagnosis and treatment plan. Our 80yr old male patient came to the opd with chief complaints of 1.fever since 7 days. 2.Dry cough since 5 days. This E log book also reflects my patient-centered online learning portfolio and your valuable inputs on the comment box.  Our 36 yr old female came to the opd with chief complaints of  1.Shortness of breath since 2 hours  2.fever with chills and rigours.& Headache.since 4 days. HOPI :- Patient was Apparently asymptomatic 4 days back then she developed fever with chills &rigours,headache.Then she was having Shortness of breath since 2 hours .She was suffering from diarrhoea since 3 days.she also had burni PAST HISTORY: - S/O mastectomy (left)1 1/2 yr back  H/O renal calculi,7 year back...

B.Shradha 1701006014 51 year old male patient who is resident of Suryapet ,and works in Good transportation company came to the hospital with  #CHEIF COMPLAINTS:- of  1- Fever since 10 days 2- Cough since 10 days  3-shortness of breath since 6 days  #HISTORY OF PRESENTING ILLNESS :- *Fever since 10 days which is high grade , with chills and rigors , intermittent ,relieving with medication. *Associated with cough and shortness of breath. *Cough since 10 days which is productive ,mucoid in consistency,whitish ,scanty amount ,more during night times and on supine position ,non foulsmelling ,non bloodstained . *Right sided chest pain - diffuse , intermittent ,dragging type , aggravated on cough ,non radiating ,not associated with sweating , palpitations. *Shortness of breath since 6 days , insidious onset , gradually progresive ,of grade 3 - (MMRC scale ),not associated with wheeze ,no orthopnea ,no Paroxysmal nocturnal dyspnea, no pedal edema . *There is history of pain...
